Appointments with Dr. Cook may be made by telephone (757-675-1720), Monday through Friday, during normal business hours (8 am until 5 pm). During non-business hours, Dr. Cook can be reached via voicemail or text messaging (757-675-1720) or email ( Unless other arrangements are made directly with Dr. Cook, appointments are routinely scheduled between 11 am and 7 pm and last for 50 minutes.

At the time a telephone appointment is made, Dr. Cook will ask for basic client information including:

In addition, Dr. Cook may ask that the patient bring copies of relevant medical and health-related reports with them to the first appointment. Please refer to Dr. Cook’s Appointment Cancellation Policy for additional information about cancelling and/or rescheduling appointments.

Dr. Cook’s services are billed at the rate of $75/hour (see policy regarding pro-rated telephone consultation rates). At the time of the first visit, and for those clients who have health insurance coverage, Dr. Cook will collect additional information (including obtaining photocopies of the client’s driver’s license and health insurance card) that will allow her to bill the client’s insurance company.

All charges not covered by insurance (co-pay, co-insurance, etc.) are due at the time of service. Dr. Cook accepts cash, personal checks, and major credit cards.

What to Expect at the First Visit

Clients are normally asked to arrive a little early for their first appointment in order to provide additional background information. Depending on the complexity of the client’s situation, additional lab work, psychological assessments or nutritional studies may be requested in support of the treatment plan. Clients may be asked to sign a Release of Medical Information Authorization form if privileged medical information is needed.

At the first visit, Dr. Cook will conduct an intake interview and perform an initial assessment of the client’s situation. A preliminary treatment plan will be developed with the client and scheduling of future appointments will be discussed. Copies of Dr. Cook’s policies and procedures will be provided and, if necessary, policy acknowledgement forms will be signed by the client(s).
